Summary

Mass-scale COVID-19 vaccination in Bangladesh—the world’s eighth-most populated and one of the most densely populated countries—is quite challenging, primarily due to the absence of a robust cold-chain infrastructure for temperature-sensitive vaccines. Any system inefficiency leads to losing the vaccine potency and efficacy. Therefore, the successful COVID-19 vaccination in Bangladesh will require massive expansion and optimal use of the existing cold-chain infrastructure that will depend critically on understanding the multi-layered vaccine supply-chain system. We conducted a nationally representative survey at different administrative levels of Bangladesh between 5 January and 11 February 2021 and collected data on the entire vaccine cold-chain system: logistics, capacity, physical assets and inventories, and human capital. Our findings suggest that, in addition to some inherent constraints, Bangladesh’s vaccine distribution system will face significant challenges, particularly from the supply-side for mass-scale COVID-19 vaccination. Rapid mass-scale COVID-19 vaccination would require significantly upgrading the cold-chain, manpower skills, and data capacity. The survey data will assist in developing a ‘digital twin’ for the vaccine infrastructure to identify robust solutions from a larger pool of complex socio-techno-economic-environmental scenarios.
